To work alongside clients, families, carers and multi-disciplinary team members to co-ordinate care, supporting collaborative decision making about care and treatment.
To deliver a set of wellbeing-focused psychologically informed interventions aligned to cognitive – behavioural principles, based on the best evidence available that address problems often experienced by people severe mental health problems and/ or often who have experienced trauma.
To work under clinical supervision from a suitably qualified professional to provide high quality collaborative treatment as a member of the multi-disciplinary team. To work under close clinical skills supervision from an HCPC registered clinical psychologist or BABCP accredited cognitive behavioural therapist to provide specified psychologically-informed wellbeing-focused interventions.

To form professional relationships with clients and communicate with them in a way that respects their views, autonomy and culture.
• To monitor and record progression, highlight and document concerns and report any perceived changes in clients wellbeing to the team.
• To demonstrate effective communication skills in discussing highly sensitive and complex information appropriately with people whose understanding may be temporarily or permanently impaired.
• To provide relevant information, written and oral, for records and reviews relating to clients progress under supervision from a suitably qualified member of the team.
• Provide highly effective written communications as required for clients and other health and care professionals.
• Report regularly and in detail to the clinical skills supervisor and line manager
• Make good use of clinical supervision and line management to discuss casework, make plans for action, reflect on and implement best practice in care planning and interventions.
• Able to assist in assessment and observation activities related to individuals health and wellbeing, arriving at judgements about how to respond within the care and treatment plan.
• Able to contribute to risk assessment drawing on complex and multiple sources of information, under clinical supervision.
• Able to analyse and synthesise multiple sources of information, to contribute to the teams understanding and formulation of clients difficulties and development of a multi-disciplinary plan of care and treatment.
• Able to assist in planning and delivering psychologically informed interventions to meet peoples health and wellbeing needs.
• Able to prioritise workload according to peoples changing needs and the priorities of the service.
• Fit to undertake any duties related to the role including physical interventions for the management of violence and aggression (Breakaway techniques only).
• Carry out relevant risk assessments and risk management with the multidisciplinary team.
• Engage calmly and with sensitivity and empathy to support clients in sometimes highly distressing or emotional circumstances.
• Engage with and gather information from clients, relatives and health records to develop a formulation.
• To work in collaboration with the individual client and their family and/ or carers as appropriate to develop care plans that are focused on strengths and are outcome based.
• To attend multi-disciplinary reviews and act as a case co-ordinator, for a defined caseload of service users in order to support and monitor progress during the course of multi-disciplinary interventions.

• To be responsive to clients needs and choices and uphold their right to be treated with dignity and respect.
• To include carers and families in line with the clients wishes.
• To liaise with other health and care providers, including third sector agencies, mental health, and Primary Care to ensure continuity of care for clients.
• To develop collaborative plans for relapse prevention.
• To deal with endings appropriately and safely with clients, families and carers.
• To ensure that up to date clinical records are maintained in accordance with professional and Trust Standards
• To complete and stay up to date with statutory and mandatory training.
