Main Duties and Responsibilities 1. To case manage those patients who require input from a specialist inpatient neurological rehabilitation facility. Once the patient is transferred to the specialist unit, regular reviews should then be completed. The post holder will also be an active lead in the discharge planning process, ensuring that the transition between environments occurs as smoothly as possible. 2. To manage a caseload with support and supervision from more senior colleagues. To maintain accurate associated records, showing evidence of good problem solving and reasoning skills. 3. To use clinical skills to guide and assimilate assessments undertaken in the acute hospitals and whilst in specialist placement to ensure that there is a full understanding of the patients clinical needs. 4. To liaise with other therapists and clinicians to ensure that the patients assessed need are being met. 5. To understand the requirements of the Mental Capacity Act and ensure that processes aligned to decision making for the pathway are being followed.. For those that are able to make informed decisions, consent will need to be obtained as indicated. For those who lack the capacity to give consent, you will need to have an understanding of ethical/legal framework when working with such individuals. 6. To attend and occasionally chair regular review meetings either via MS teams, teleconference or face to face as required 7. To complete comprehensive risk appraisal procedures and formulate action plans to ensure that the patient and/or carers are as safe as possible in their current or future environments. 8. To support the provision of Environmental Controls in conjunction with the lead therapists, undertaking assessments for specialist systems and advising on the use of mainstream technologies as appropriate. 9. As part of Bedfordshire Neuro Therapy Services there will be a very occasional requirement to cover other pathways as required. To contribute to the provision of rehabilitation programmes to patients on other pathways through specialist assessment and rehabilitation intervention. To identify and review rehabilitation goals which the Rehabilitation Technician will support. 10. To contribute to the development of information sheets and service leaflets 11. To ensure that services are provided in accordance with the Trusts standard procedures and objectives of quality assurance and they are consistent with best professional practice. 12. To monitor the quality of services delivered by partner agencies commissioned to provide specialist rehabilitation to the caseload. To raise concerns and escalate appropriately if agencies do not meet treatment/care standards e.g. Safeguarding as appropriate. 13. To undertake service evaluation and development with support from the multidisciplinary team. 14. To identify unmet needs and service gaps within the pathways provided by Beds Neuro Therapy Services. Identified issues should then be relayed to the head of service. 15. To contribute to the development and revision of ABI Service procedures and guidelines. 16. To be involved in the recruitment process of other members of staff as appropriate. 17. To participate in staff and service development through engagement in service meetings and in-service training sessions including presenting sessions as appropriate. 18. To be responsible for maintaining own competency to practice through clinical professional development (CPD) activities and maintain a portfolio that reflects personal development. 19. Develop and maintain a sound knowledge of evidence based practice in the relevant clinical field and implement this into every day practice. 20. To plan and prioritise ones own workload, balancing patient related activities with other professional demands. 21. To be able to practice within a multidisciplinary team, accessing supervision and guidance from other members of the clinical team. Communication 1. To provide the necessary emotional support to neurological patients and their relatives who may be experiencing distress as they adjust to their disability, or their family members disability. 2.To demonstrate the ability to deliver difficult information to patients/carers with tact and empathy. 3. To be aware of and support communication difficulties with patients who present with a range of physical and psychological barriers to engagement in treatment (e.g. dysphasia, poor memory). 4. To promote and maintain effective communications and good working relationships with a range of stakeholders involved in the care of Bedfordshire patients with neurological disability. 5. To escalate and support any conflict that develops as part of the case management process. 6. To plan and convene patient case conferences and liaison meetings as indicated. Budgetary Responsibility 1. To consider eligibility for NHS Continuing Healthcare Funding (CHC) of patients with a neurological condition. To complete NHS CHC Checklists in a timely manner and liaise with the CHC Department as indicated. 2. To identify clinical need in order to assist in the negotiation of financial issues related to the placement and funding of individual patients 3. To provide responsible prescription & authorisation of equipment and services for neurological patients within the resources and budget available. 4. To make robust applications to Individual Treatments Panel the CCG when requesting funds for specialist rehabilitation. These requests should be supported by a clear clinical rationale and evidence of rehabilitation goals. 5. To represent BLMK ICB as funders when attending rehabilitation reviews during specialist rehabilitation placements. To ensure specialists placements provide effective clinical treatments which are of high quality and value for money. To make recommendations to providers and commissioning colleagues to support this process. People Management To engage in and support joint working and peer supervision with colleagues from Bedfordshire Neuro Therapy Services. To provide clinical training and supervision to staff members of Bedfordshire Neuro Therapy Services as appropriate. To liaise with other team members to organise and appropriately escalate clinical caseload as appropriate/required. 4. Maintaining own professional development and requirement to take part in appraisal and KSF process Clinical and Practice Governance 1.Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act. 2. Any data that is taken/shared as part of a phone call or transported, faxed, or transferred electronically must be undertaken with regard to the Trust Information Governance and Information Security policies. 3. The post holder must adhere to the Trust risk assessment and risk management processes 4. The post holder must adhere to infection control policies and procedures 5. It is a condition of your employment that you are currently registered with HCPC and it is your responsibility to maintain your professional registration. 6. Undertake mandatory training and any other training relevant to the role as required by Cambridgeshire Community Services NHS Trust. 7. The post holder must participate in clinical and other audits as required 8. Provide patients and their families /carers with information on standards they should expect from the team 9. Participate in clinical supervision on a regular basis 10. The post holder is required to participate in relevant emergency preparedness process for their team.
