As part of our carer support team, your key focus will be supporting the rest of the team and providing services directly to carers and in partnership with multiple organisations and community groups.
You will be responsible for providing carers with information and advice, signposting additional services, breaks from their caring role, and being someone to talk to. You will have specialist knowledge in young adults with a view to providing tailored support to young adult carers. You will carry a caseload of carers who need support with managing their caring role. You will support making referrals to relevant organisations. You will work in partnership with other stakeholders to empower young adult carers to achieve their goals, particularly around their transition into adulthood. You will deliver this service via a varied platform including by phone, text, email, online, and occasional face to face.
Key accountabilities of the role
* To carry out carer conversations and action plans together with adult carers and take a lead on young adult carer cases.
* Work together with young adult carers (and where appropriate, their referrer) to ensure a smooth transition from young carer services to adult carer services.
* Use a person-centred approach to explore the needs identified within a transition assessment, or action plan, and work together with internal Caring Together teams to refer to identified services.
* Provide relevant, accurate and up to date information, advice and guidance, including making onward referrals and introductions as needed.
* Maintain up to date awareness/knowledge of provisions of services/support available and offer the best service to carers and to share with the team to continue to build internal resources/information files.
* Work together with external stakeholders, such as but not limited to, housing associations, mental health support providers, education and employment settings, to support the outcomes as identified by the carer.

Skills and knowledge
* Passionate about people and experience of delivering high quality support focused on the needs of individuals.
* Understand challenges facing carers, and their support needs.
* Excellent written and verbal communication skills, disseminating information widely to a diverse range of carers and stakeholders.
* Ability to prioritise tasks and respond to changing work priorities.
* Effective administrative experience with excellent IT skills. Experience in Microsoft software such as Word and Excel.
