To provide general information and support about cancer and palliative care services, to enable patients to navigate the health and social care system and make choices that are best for them personally.
Responsible for proactively identifying patient and carer needs using knowledge approved tools and procedures to ensure patients get the right support to meet their needs, providing appropriate advice and escalating to the registered practitioner where and when appropriate.
In conjunction with the Cancer Nurse Specialist (CNS) and team members, coordinate access to the right information and education resources to support patients throughout the complex cancer pathways.
Develop a partnership approach to working to empower the patient and carers.
