Recovery Programmes Practitioner

Salary: £24,000 per annum

What you will be doing?
1. You will be working within an integrated healthcare framework to provide support to prisoners aged 18 and over.
2. Services are designed to deliver psychosocial drug and alcohol interventions and accredited treatment programmes for offenders coming into prison who are identified as having an alcohol or drug problem.
3. Services will deliver client-centred treatment using both harm reduction and abstinence-based treatment approaches depending on the requirements of the individual client.
4. You will be required to manage a caseload, provide appropriate harm reduction advice and guidance, assess, care plan, and deliver structured 1:1 and group-work sessions including structured treatment programmes relevant to the unit you are working in.
5. Through care and integration with other services such as Transitional, Housing, Employment, Gym, Healthcare, Probation, and external Drugs Intervention Teams, you will prepare clients for release.
6. You will be responsible for contributing to the targets set by Forward and the local commissioners, including measuring outcomes and maintaining cooperative working relationships with partner agencies.
7. Flexibility will be required in this role, which may include working on other projects in your cluster to deliver services, enabling continuity of care for the client group.
What are we looking for?
Our core purpose is to break the cycle of crime, and you play a key part in bringing it to life. This means delivering tailored interventions based on your clients' individual requirements, thinking collaboratively, and providing a seamless link into our clinical, family work, housing, employment, and recovery and reintegration services, as well as collaborating with other agencies.
The ultimate aim is for our clients to successfully reintegrate back into society with plenty of recovery and wellbeing capital to build on.
Day to day, you will deliver structured 1:1 and group-work sessions for both Substance Misuse and Low Level Mental Health service users to provide transitional support from prison to the community in preparation for their release.
Our success is based around targets and measuring the effectiveness of our work, so accurate and timely recording of data will be a vital part of your role.
It is desirable that you can speak Welsh, but not essential. This is to support requirements of the contract.
