Job Title:
Lead Worker/Support Worker
Hours of work:
Full Time (37.5 hours per week)
Key Responsibilities:
* Undertake holistic risk and needs assessments within the community on an outreach basis.
* Develop support and risk management plans using appropriate assessment tools, such as the Outcome Star.
* Ensure a non-judgmental, person-centred approach at all times, utilising various approaches to promote engagement.
* Work with young people with complex and multiple needs to help them sustain tenancies and manage their lives better.
* Support and encourage young people to engage with learning skills, apprenticeships, traineeships, and volunteering opportunities.
* Support and encourage young people to develop positive family and other support networks.
* Mediating between young people and family/friends to develop positive relationships and increase support networks.
* Assist young people in identifying and taking up settled accommodation and provide ongoing support.
* Work with young people and employers to achieve and maintain employment.
* Be available for regular support sessions and respond to crises.
* Take responsibility for safeguarding young people's welfare.
* Promote and facilitate young people's participation in their support plans and service management.
* Facilitate access to statutory and non-statutory support services, including health, mental health, and substance misuse services.
* Act as a link between the client and other services to ensure coordinated responses.
* Advocate for young people on issues like benefits and housing rights.
* Support young people in understanding their rights and responsibilities.
* Ensure benefit claims are made and maintained, support budgeting, debt advice, and small grant applications.
* Help young people improve life skills through practical support like shopping and cooking on a budget.
* Scope out local services and support young people in connecting with the community.
* Organize and accompany young people to appointments as needed.
* Maintain accurate records, including support plans and outcomes.
* Manage a caseload independently, prioritising workloads and managing risks.
* Assist in induction of new staff, supervise and support volunteers, trainees, and students as required.
* Work as part of a team, providing cover for colleagues when needed.
Qualifications:
* GCSE A-C in Maths and English or equivalent
* NVQ 3 in Advice and Guidance, Health and Social Care, or relevant subject
* Youth Work or Social Work degree
Role Experience:
* Experience in Youth Work, Residential or Social Work, Probation/Criminal justice, Community Work, Education and Training, housing, or support work.
Knowledge:
* Up-to-date knowledge of welfare rights and resources
* Understanding of the complexity of needs faced by young people
* Knowledge of relevant support services, including Mental Health, Social Care, Health, Substance Misuse, CSE, and County Lines
* Familiarity with relevant legislation such as Housing, Homelessness, Children’s and Care Acts
Skills:
* Ability to proactively seek out services to meet young people's needs
* Ability to relate to and engage with young people with vulnerabilities
