Job summary
Pike View - Dr Malhotra & Partners is a GP surgery located in the new Horwich Health & Wellbeing Hub with just over 9,000 patients.
The right candidate will help us to continue delivering safe and co-ordinated care to our patients with focus on chronic disease management, cytology, immunisations, contraception (pill/depo)

Main duties of the job
Implement and evaluate individual treatment plans for chronic disease patients
Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health
Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
Support the team in dealing with clinical emergencies
Implement vaccination programmes for adults and children
Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
Support the clinical team with all safeguarding matters, in accordance with local and national policies
Deliver opportunistic health promotion where appropriate

We are a long standing high achieving practice in QOF & BQC
Our clinical system is EMIS alongside Docman 10, Ardens, Accurx, Iplato.
Due to a recent move to a brand new purpose built A+ energy rated building we are expanding our clinical and administrative team.

Job responsibilities
Job responsibilities include but not limited to:
1. Develop, implement and embed health promotion and well-being programmes
2. Implement and evaluate individual treatment plans for chronic disease patients
3. Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health
4. Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
5. Provide travel medicine services
6. Request pathology services as necessary
7. Process pathology results as required
8. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required
9. Maintain accurate clinical records in conjunction with extant legislation
10. Ensure read codes are used effectively
11. Maintain chronic disease registers
12. Chaperone patients where necessary Prioritise health issues and intervene appropriately
13. Support the team in dealing with clinical emergencies
14. Implement vaccination programmes for adults and children
15. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice)
16. Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
17. Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
18. Support the clinical team with all safeguarding matters, in accordance with local and national policies
19. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
20. Deliver opportunistic health promotion where appropriate

Qualifications
Essential
21. Current valid NMC registration
22. Previous practice nurse experience
Desirable
23. Cytology smear taker
24. Immunisation training
25. confidence in managing CDM
