We are seeking a Mobilization Manager to play a key role in the development and project management of new care packages for adults in supported living settings. About the Role As a Mobilization Manager, you will be responsible for overseeing the successful transition of individuals into bespoke supported living services. You will work closely with multidisciplinary teams, external care providers, registered providers, and family members to ensure smooth and well-coordinated care arrangements tailored to the needs of each individual. Key Responsibilities: ✅ Lead on the development and mobilisation of new care packages, ensuring they are person-centred and meet individual care needs. ✅ Conduct and support referral assessments, working collaboratively with local authorities, commissioning teams, and other stakeholders. ✅ Engage with external partners, including multidisciplinary teams, housing teams, estates, and registered providers, to coordinate seamless transitions. ✅ Oversee property adaptations and ensure supported living environments are suitable and meet the specific needs of individuals. ✅ Work closely with operational teams to ensure care services are fully prepared for new individuals, including staffing, training, and compliance requirements. ✅Ensure a high standard of service delivery, meeting regulatory and best practice guidelines. About You Proven experience in mobilizing care packages or project management within adult social care, supported living, or a related field. Has worked in as a Registered Manager/Service Manager/Deputy Manager level, supporting Adults with Learning disabilities and additional needs. Strong knowledge of person-centred planning and best practices in adult supported living services. Excellent stakeholder engagement skills, with experience working alongside multidisciplinary teams, local authorities, and housing providers. Ability to oversee property adaptations and ensure appropriate supported living arrangements.
