Job summary We are recruiting for a Lead Clinical Nurse Specialist - Palliative Care who share our vision to be trusted to provide consistently outstanding care and exemplary service to our patients. This is an exciting opportunity for an experienced Band 7 Clinical Nurse Specialist working at an advanced level to be involved with the continued service developments within Cancer Services and Palliative care. This senior nurse will provide expert palliative care support to both inpatients on our unit and outpatients attending for treatment. Main duties of the job Responsible for assessing, planning, implementing and evaluating care as an autonomous practitioner with specialist skills. Provision of care through accepting referrals, physical assessments and support on the inpatient unit, nurse-led clinics within the outpatient setting. Referral of patients directly to specialists in other professions Allocation of cases to other members of the specialist team Provide a high level of communication by liaising with all appropriate members of multi-disciplinary team and ensuring a seamless service between acute and primary care services. Provide expert specialist education for both staff and patients Maintain own knowledge, competence and skill level through regular clinical supervision, appraisal, personal development plans and appropriate training programmes.
