We are looking for a highly motivated and skilled Assistant Practitioner to join our Frailty team. This is an exciting opportunity to be part of a specialized service that focuses on improving outcomes for frail patients, helping them manage complex needs and maintain their independence.
Main duties of the job
As an Assistant Practitioner, you will work alongside a multidisciplinary team of nurses, doctors, physiotherapists, and occupational therapists, contributing to the care and rehabilitation of frail, older patients. You will play a vital role in supporting the assessment, planning, and delivery of patient-centred care within a hospital or community setting.

Person Specification
Qualifications
* QCF level 5 in Health & Social Care or to be able to complete this once in post in a timely manner
Experience
* Relevant and relatable experience at a Band 3 Rehabilitation Assistant level.
* Evidence of reflective practice and commitment to continuing professional development.
* Evidence of experience working with patients living with Frailty.
* Experience in the appropriate clinical area.
