West CMHT (this post): Older people CMHT service for Southwest York Locality as well as Selby has an over 65 population of about 22000. The CMHT provides Monday to Friday 9am - 5pm assessment and treatment services which include the provision of care for older people with functional mental health problems as well as complex organic care for patients in their own homes. The entire southwest team receives about 400 referrals each year. The approximately the West team receives around 50% of the southwest team referral. The post holder is expected to see approximately 6 to 8 new CMHT patients a month. The team implements purposeful and productive community service model. The postholder is expected see approximately 8 to 10 review CMHT patients a month. This post has 1.5 PAs of Memory clinic work inbuilt to make it a fulltime post. The post holder is expected to see around 9 new memory patients in every 2 weeks period. The memory initial assessment and cognitive testing are completed by Memory Nurses. The post holder is not required to review previously seen memory service patients. Memory Monitoring nurses review patients who have previously seen the consultants. At the time of writing this there are no CTO or section 37/41 patients. Usually there is no more than 1 or 2 patients a year per consultant under MHA.
Main duties of the job
This is a replacement post. It is a full-time post. However, less than full-time applicants are encouraged to apply. The Consultant will provide input working to a 7.5 direct clinical care (6 DCC to West CMHT and 1.5 DCC to Memory Team) and 2.5 PAs supporting professional activities for the West team.
There are seven other MHSOP Consultants in the York and Selby area. One Consultant covers Hospital liaison. There is a dedicated Consultant to the Moor Croft Ward and a similarly dedicated Consultant for the Wold View ward. There is a dedicated Consultant to the York and Selby Memory services. The two Community Mental Health teams have support from 4 Consultants i.e., East 1 WTE, North 0.75 WTE, West 1 WTE(this post) and South 0.8 WTE. There is a dedicated 1 WTE Consultant for York Older Adults Liaison team.
