Main duties of the job
Nurse- Sutton Health and Care at Home Band 6
We are looking for an enthusiastic Nurse to work as part of the Sutton Health and Care- At Home service. You will be a part of the MDT team that helps to prevent admission from community, accelerate discharge from emergency department and acute medical wards and provide supported discharge through discharge to assess pathway.
We are innovative and people focused with a 'one team' ethos, which means better quality and more joined up care for our patients and families. The team strives to constantly improve service delivery and contributions from all staff are highly valued.
We will provide professional development opportunities and support you to provide great care to our patients. Our services operate 8 am to 8 pm seven days a week. You will have opportunities to do additional hours on evenings and weekends. We have pool cars for community visits.

Education/ Qualifications
Essential criteria
1. RN (adult) with NMC Registration
2. Relevant post registration study at degree level and relevant CPPD
3. Mentorship course or equivalent
Desirable criteria
4. Advanced physical assessment or working towards
5. Independent prescriber or working towards
6. Extended Nursing Skills or working towards
Experience
Essential criteria
7. Experience of conflict management
8. Experience in managing the needs of complex patients in relevant care setting, i.e. community or unscheduled care
9. Care Planning
Desirable criteria
10. Experience in supervising, mentoring and appraising staff
Skills/Abilities/ Knowledge
Essential criteria
11. Holistic Assessment skills
12. Effective clinical skills
13. IT skills including keyboard and internet skills
14. Able to use email, carryout research and access e-patient records, access policies and procedures
