Job summary The Community Learning Disability Team has a vacancy for a Band 6 Nurse. This role will support people with a learning disability who are placed out of area, and work with people in the community who require nurse input. The Learning Disability Team is a multi-disciplinary team who are passionate about providing the best possible care. Main duties of the job The post holder will be responsible, together with the wider team, for providing care, interventions and health promotion to people with learning disabilities. It is essential that robust communication networks with clients, carers and other health workers and care agencies are promoted. Utilise a range of communication skills to ensure that clients with learning disability understand the implications of their health issues, including complex health problems. Utilise a range of communication skills to assist capacity for consent to treatment and provide advice regarding best interests when a client lacks capacity. Maintain a knowledge / skills base and evidence the same through a personal professional portfolio and act in accordance with the NMC code of professional conduct. The post holder will also undertake a supervisory role in respect to multi-disciplinary junior team members and work alongside the Team Leader on clinical issues such as therapeutic approaches and care co-ordination. This role will lead on supporting people with a Learning Disability who have been placed out of area.
