We're currently recruiting Remote Advanced Clinical Practitioners (ACP) and Advanced Nurse Practitioners (ANP) from a variety of professional backgrounds including nurses, pharmacists and paramedics - if you're experienced, with a Masters level award or equivalent, we would welcome your application. As an Advanced Clinical Practitioner within our Remote Team, you'll be expected to assess all patients of all ages and provide the right management or referral. You'll have to be reliable and dependable, passionate and committed to delivering high quality health care in a telephone assessment environment. We operate CAS telephone-based services within our Contact Centres in Newcastle, Teeside & Staffordshire and a home-based team. You will be part of the multidisciplinary team which includes GPs; ACPs, UCPs and Prescribing Pharmacists. Patients will be referred via NHS 111 and Ambulance services where you will be expected to use your clinical expertise to treat, advise or refer them on to the most appropriate service. Your day to day As an Advanced Practitioner, you will be demonstrating advanced clinical competence and a knowledge base beyond those associated with conventional clinical roles. Undertake consultations, via telephone triage, with patients of all ages (neonate to elderly) as an autonomous practitioner and using own clinical judgement to diagnose, treat, refer and/or discharge patients. Make direct referrals to other and alternative services within locally agreed pathways, guidance and protocols. Taking responsibility and being accountable for managing own workload, risk assessment and management, acting within professional codes and appropriately to the clinical team leader for advice and support. Prescribe, supply and arrange administration of medicines as indicated to address patient need via electronic prescribing.
