We operate services across Bournemouth, Christchurch and Poole, ranging from Residential Homes, Day Services, Supported Living sites and Reablement Home Care.
The Role
As a Community Support Coordinator you will be responsible for the planning and coordination of housing and community outreach support visits to maximise contact time and facilitate the support of new clients when required.
The Community Support team provides housing related support to individuals living in supported accommodation. The service assists people to maintain their independence within the community, preventing loss of their home or tenancy by supporting individuals to gain, re-learn or maintain life skills in order to carry out their own day to day tasks, such as cooking, cleaning, laundry, budgeting and paperwork.

Your Main Responsibilities
* Oversee the scheduling of staff team providing housing and community support, ensuring efficient and effective use of resources.
* Coordinate visits to minimise travel time and maximise visit capacity.
* Maintain clear and effective communication with clients, families, and healthcare professionals to ensure a coordinated approach to planned support visits.
* Support with reception and answering telephone calls.
* Ensure the smooth delivery of community and housing related support services with advance planning of rosters.
