We have an exciting opportunity for a Band 7 Manager for an 18-month pilot, funded by the EmpowerED Lead Provider Collaborative in line with the move towards 'Alternatives to Admission'.
PARALEL is a new innovation, designed as a 'step-down' intensive community service for patients discharged from Oaktrees Eating Disorder Unit (EDU) for a period of 12 weeks (when relapse risk is at its highest).
The B7 manager will be tasked to build a workforce, develop a clinical model and implement this model across the CWP footprint. This will likely be a combination of virtual and face to face work. The successful candidate will also be tasked to train and supervise other members of the team, as well as leading on collecting outcomes for the project.
Main duties of the job
The clinical work will involve liaising closely with Oaktrees EDU to identify patients close to discharge, developing a discharge plan with realistic goals, working in a rehabilitative approach to improve motivation to continue recovery, improving life domains, and transferring skills learnt on the ward into their home life, as well as nutritional support. They will also be heavily involved in carer support.
This will be an enhancement to treatment-as-usual through the community eating disorder service, therefore there will be no obligation for the PARALEL team to manage risk or medical interventions, deliver therapy or care co-ordination. However, there will be an expectation to work and communicate closely with the community team to enhance treatment and overall service delivery.
Through a rehabilitative, holistic approach and appropriate assessments and care planning, the successful candidate will be able to take on the exciting challenge of managing a complex case load and improving the quality of life of service users as well as development of a new service. Part of the role may also involve group work which is an integral part of this service.
Likely outcomes will be reducing readmission rates, reducing overall length of stay, improving transition from inpatient to community, increasing patient contact during this vulnerable period and improving patient experience.
