Person Centred Review and Individual Development Plan Officer
Salary: £39,513.00 - £43,693.00 per annum
Permanent full time 37.00 hours per week.
We are currently looking for a suitably qualified and experienced person to join our team.
The successful candidate will be responsible for:
1. Supporting the Local Authority (LA) in the discharge of its duties under the Additional Learning Needs Education and Tribunal (ALNET) Wales Act 2018 and the accompanying Additional Learning Needs (ALN) Code for Wales 2021.
2. Supporting the LA with the development, management and quality assurance of Individual Development Plans (IDPs) as set out in the ALNET Wales Act 2018.
The main duties of the post include:
1. Assisting the ALNSS Manager in coordinating and discharging the LA's duties in relation to the development, management and quality assurance of Individual Development Plans (IDPs).
2. Organising and leading Person Centred Review (PCR) meetings between the learner, parents/carers and all agencies involved with the young person to create a fully integrated IDP between all parties.
3. Preparing high quality Local Authority Maintained IDPs, which meet all legal requirements, in accordance with the statutory advice received, within the appropriate and applicable timescales.
4. Participating in the quality control monitoring of the format of completed IDPs.
You will have experience in:
1. Conducting Person Centred Reviews and developing plans for Children and Young People with ALN.
2. Working with settings and schools.
3. Collaborating with parents and agencies.
Ideally, you will have:
1. A degree or equivalent and/or ability to demonstrate competence through experience.
2. Accreditation in Person Centred Practices (PCP).
