An exciting opportunity has arisen for an enthusiastic and motivated Band Deputy Home First Co-Ordinator to join the multi-disciplinary Home First Team based in Wrexham. Now is a great time to join the Home First Team, as we further expand and develop the service to allow more and more patients to be cared for at home.
The post is hours covering days a week between am and pm, to meet the service requirements.
The successful candidate will be deputise for the Home First Co-Ordinator.
Main duties of the job
They will also be responsible for the ongoing screening of patients and the identification of care availability to facilitate safe and timely discharge from hospital and provide ongoing support to any ongoing or complex needs within the community through the Discharge to Assess model of care. The successful post holder will work within a supportive multi-disciplinary team to promote the Home First ethos through ongoing therapeutic interventions in the patient’s own environment and on the Ready for Home wards.
The successful candidate will act as a link between the acute and community services and will work alongside our established Community Resource Team’s in both Wrexham and Flintshire.
