NHS Somerset Integrated care board (ICB) have a vacancy foran experienced pharmacist to join us as a Band 8b Locality Medicines ManagementPharmacist. Specifically, you will:
provide expert advice to clinicians within a locality(primary care network/s) of 10-15 GP practices on Medicines Optimisation,including policies and procedures to achieve high quality, safe, effectiveprescribing, and value for money
facilitate medicines optimisation between patients,prescribers, community pharmacies and secondary care, including providingadvice on the Community Pharmacy Contract and commissioning of pharmacyservices
assist the Chief Pharmacist in advising Somerset ICB andwider Somerset System on Medicines Optimisation issues including health servicedevelopments, governance, professional issues, formulary development,introduction of new drugs, patient complaints, pharmacy services.
Main duties of the job

KEY RESPONSIBILTIES OF THE ROLE:

Providing advice on the commissioning ofpharmacy services
Working with the Associate Director ofMedicines Management to advise Somerset Integrate Care Board (ICB) on MedicinesOptimisation issues including Health Service Developments, Governance, Professionalissues, formulary development, introduction of new drugs, patient complaints,pharmacy services
Overseeing and managing the implementation ofspecific changes in prescribing medication, formulary changes and medicationreviews within General Practice by employed and commissioned staff
Meeting at least annually with each GPpractice within their locality and to provide support and influenceimprovements in the quality, safety and cost effectiveness of medicinesoptimisation
Providing data to individual practices andfederations to assist with peer comparison of medicines optimisation data
Leading on the development and implementationof specific projects as delegated by the Associate Director of MedicinesManagement
To lead onthe cost effectiveness of the commissioned practice support pharmacists andtechnicians
To provideexpert advice to commissioning working groups on areas of medicines involvementin pathways
Providing expert knowledge and input intodelegated committees and patient pathway groups e.g. drugs and therapeutics committees
To manage the improvement in the quality,safety and cost effectiveness of prescribing by performance monitoring theimplementing specific changes in prescribing medication, formulary changes andmedication reviews within the locality group of General Practices
To work closely with the practice manager andprescribing lead of each practice including where possible regular discussionswith individual practices within the federations about the annual QOF and LocalEnhanced Service targets. Monitor andwork with the practices to ensure the targets are achieved
To support the implementation of the ICBmedicines optimisation strategy to deliver better clinical outcomes forpatients and lead on specific areas as delegated by the Associate Director ofMedicines Management
To facilitate the training and improvement ofpractice staff and support staff, including leading on specific trainingsessions, contributing to the newsletter and presenting at prescribing leadsconferences.
To ensure that CMO/Medicines and HealthcareProducts Regulatory Agency (MHRA) and other Drug or safety alerts are reviewedwithin the practices and federations
To ensure all support staff communicateeffectively and successful with patients on medicines optimisation includingchanges to their medication including clinical and cost effectiveness benefits
