This is an exciting opportunity to join the Kent Health Visiting Service as a Family Partnership Practice Lead (FPPL) to work within the Maidstone District. The FPPL has a key role in embedding the Family Partnership Model within Kent Health Visiting Services under the guidance of the Family Partnership Programme Manager. The Family Partnership Model is an innovative approach which is evidence-based and enables parents and families to overcome their difficulties, build strengths and resilience, and fulfil their goals more effectively.
Working as part of the local Health Visiting team, the Family Partnership Practice Lead will facilitate the early identification of vulnerable families and support the delivery of the KCHFT Family Partnership Programme by managing a small caseload of families, supporting them ante-natally and until their infant is a year old.
The post holder will also have a lead role in embedding the Family Partnership Model within district health visiting practice as well as providing leadership and some line management responsibilities to the team.
The work with clients is intensive and challenging but provides a real opportunity to make a difference to their lives. You need to be a resilient and flexible Health Visitor with excellent communication and team leadership skills.
