Responsibility for the promotion and use of the audit cycle within clinical practice.
Support the development and implementation of Healthcare Policies and procedures.
Responsibility of offering the initial assessment/completion of the “All Wales Blue book” to Asylum seekers living in ABUHB Locality.
Responsibility of providing cultural awareness training and needs of the socially vulnerable groups to other disciplines such as GP practices staff, mental health teams, and Accident and Emergency staff.
Work in conjunction with other disciplines in the assessment, planning, implementation, and evaluation of healthcare service provision to ensure a smooth patient pathway through the services.
The post holder is responsible for the delivery of nursing practice within the socially vulnerable communities and ensuring that everyone is treated with dignity and needs are met in a timely manner.
The focus of the role is to bridge the gap of health inequalities and provide education to the communities and professionals of the social determinants within vulnerable groups.
To develop and maintain close relationships with Primary and Secondary Mental health teams and staff from a range of statutory and non-statutory services, providing advice and support to promote the management of those with mental health needs.
As an autonomous practitioner, the post holder is responsible for the care delivered, demonstrating critical thinking, sensitivity, and skills in clinical decision-making in the management of patients.
You’ll ideally have previous experience in a community setting with a knowledge and understanding of culturally sensitive trauma-informed approaches.
