To work as part of a multidisciplinary team providing a high quality service to children, young people and young adults with life shortening and life threatening conditions with complex health needs, and support their families and carers in the delivery of that care. To work as part of a multidisciplinary team providing high quality palliative and end of life care. To assess, plan, implement and evaluate individualised holistic care packages without direct supervision over a 24 hours period for the children, young people and young adults, aged 0 to 25 years, who access the service at St Oswalds. To contribute to interagency and inter-professional collaboration and team work to ensure that delivery of care is timely, well co-ordinated, effective and appropriate to need.
