The Community Resource team Home Care (CRT) is part of Cardiff Council’s Independent Living and Community Care Service (ILS) and works in partnership with vulnerable adults to support them to live independently at home and connected to their communities.
CRT is a joint service provided by Cardiff Council and Cardiff and Vale University health board that aims to support adults, through therapy and/or home care, to recover or maintain their ability to live independently at home. The service provides reablement care which aims to encourage and support people to learn or re-learn skills necessary for daily living, following a period of illness or after a stay in hospital.
About the job
An exciting opportunity has arisen to join Cardiff’s CRT Home Care team.
You will be dealing with all referrals to the team and providing support to members of the public and professionals over the telephone or electronically where required, such as signposting and completing onward referrals to other services.
You will be inputting data and using the department’s computer client record system. You will also produce basic reports in relation to the function of the team, providing statistical information as required and compile customer satisfaction questionnaires and compliments.
You will establish and maintain effective administrative systems in respect of the Community Resource Team, maintaining all team correspondence.

What We Are Looking For From You
You will have experience of working directly with members of the public/other agency personnel.
You will be IT literate, with accurate keyboard skills and excellent communication skills.
You will have the ability to work autonomously and as part of a team.
You will have the confidence to deal with customer enquiries and complaints diplomatically and adeptly.
This role will include weekend and evening working.
The ability to speak other languages, in particular Welsh, and Community Languages would be an advantage.
