Whilst this role is based at Orchard, the successful candidates will work closely with other PCN staff, including the CCO at Park Surgery. We have a higher than average population of elderly patients, many of whom live in their own homes and need a lot of social and clinical support. As a PCN and practice, we will be focusing on developing a Frailty programme for these patients, to take a more proactive preventative approach to caring for them. We also have a focus on Women's health and are working in our local community to improve access for women through Information Sessions. We have a strong focus on dementia, young people's mental health and are keen to develop these further. The PCN personalised care team will have the full support of a clinician experienced in supporting the personalised care roles and will receive monthly individual clinical supervision as well as group learning. There will be opportunities to undertake the 2 day Personalised Care Institute accredited care coordinator training and to achieve the competencies set out in the NHSE Care Coordinator Development Framework.
Main duties of the job
* The Care Coordinator is central to the multidisciplinary team within the practice and our Primary Care Network. They will be the first point of contact for patients and service users with complex needs, long term conditions or frailty, providing an in depth knowledge of practice and local community services. The Care Coordinator will liaise closely with their clinical team, the personalised care team and management teams to ensure patients receive timely and appropriate direction or appointments according to their healthcare need. Responsible for ensuring cohorts of our population receive appointments in practice or at home for monitoring of long term health conditions. This aspect of the role may include other administrative duties as requested by the management team or partners. The care coordinator will be central to the Frailty service offered by our PCN working closely with Park Surgery Care Coordinator, our PCN management team, the Clinical Directors and clinicians. In our new PCN, service development support will form a key part of the role.

Orchard Surgery is a long established small friendly practice in the centre of Horsham. We have a population of around 9000 patients. We have a strong team of GPs, paramedics, clinical pharmacists, social prescribers, health and wellbeing coaches along with supportive managers and administrative staff. Job responsibilities
* First point of contact for patients with complex needs, long term conditions or frailty ensuring that each patient is treated as an individual providing personalised care and support.
* Develop and maintain a clear and up to date knowledge of the local services, patient pathways and availability to effectively connect patients to local statutory and community services.
* Develop excellent working relationships with all partners, wider service networks including the voluntary sector, adult and children's social care, hospitals, community pharmacists and other members of the Network.
* Helping patients navigate the NHS and Social services.
* Key member of Frailty multidisciplinary team, ensuring team works effectively together through effective communication.
* Diarising meetings, minute taking and ensuring actions are progressed.
* Compiling audits, searches, monthly trackers and providing reports for the practice and PCN (training provided).
* Monitoring long term condition and medication review appointments, ensuring practice or PCN standards are met.
* Ensuring financial information is recorded and filed correctly.
* Diary management specialist clinics in practice and PCN.
* Ensure urgent referrals, telephone calls, e-mail or written queries are answered within the agreed time and in line with the Practice policy.
* Respond to enquiries in a courteous and efficient manner both face to face and over the telephone.
* Provide accurate input and maintenance of patient records and confidential data onto relevant Practice IT systems, making amendments as necessary.
* Ensure that Practice and Network policies are adhered to at all times.
* Work with practice and PCN multi-disciplinary teams to develop services.
* Participate in network meetings.
* Undertake any other duties appropriate for the role to ensure the smooth and efficient running of the practice and PCN.
* Undertake any training as required to complete the tasks associated with the job role.

Person Specification
Experience
* Proven experience of working in an administrative role demonstrating evidence of:
* 1. competent data collection and data quality.
* 2. working with confidential documents and information.
* Experience of working in a customer focused environment demonstrating the ability to deal with difficult customers and challenging situations.
* Experience of working both autonomously and in a team.
* Knowledge of medical terminology.
* Experience of an administrative role within the NHS.
* Knowledge of Practice systems (i.e Systmone or EMIS).
Qualifications
* Good general education with English and Maths to GCSE standard or equivalent.
* Relevant NVQ 3 or equivalent qualification or experience.
* IT package qualification / training such as the European Computer Driving License (ECDL).
