An exciting opportunity to join our new service that is going to work alongside our well established heart failure team.
We have commenced a cardiology virtual ward. It is based in South Derbyshire and Derby city for heart failure and cardiology patients to be closely managed and supported at home rather than being in an acute hospital bed.
We have recently appointed 3 highly motivated cardiology Advance cardiac practitioners and are looking for another to join our very enthusiastic and busy team either Band 8a (if all criteria met) or Band 7 (as a training post).
We are a proactive, integrated team that already works across Acute and Community settings and this post will complement care for patients.

The service operates over 7 days. This includes BH and weekends. This is approximately every 4-6 weeks.

Main duties of the job
This post is multi-faceted and involves working predominately within the Royal Derby Hospital, the community and the ambulatory heart failure unit.
To work as an advanced cardiac practitioner within the cardiology virtual ward /ambulatory heart failure unit and community setting.
Ensuring the delivery of high quality care, making critical decisions to diagnose conditions through clinical assessment and ordering and interpreting of diagnostic tests.
Within the acute trust, you will be assessing and reviewing patients who have been admitted to hospital with heart failure and cardiac problems, offer management advice and decide who may be suitable for having their care remotely. You will review patients within the ward, medical SDEC and the emergency department.
In the community (home visits), patients will be assessed for their suitability for the virtual ward reviewed and plan their ongoing care.
You will also be working within the Ambulatory HF unit is where we deliver some our IV therapies (iron and diuretics).
We work closely with the cardiac physiologists for device management, will be having daily MDT's with cardiology consultants and have weekly MDT's with the heart failure cardiologists at Royal Derby Hospital.

Qualifications
Essential
* Professional registration with NMC or HCPC
* Evidence of post-registration study at undergraduate level (Level 6) within cardiology and heart failure
* Independent Prescribing or study of pharmacology & therapeutics at post-graduate level (Level 7)
* Advanced clinical diagnostic skills, blood tests and interventions ordered and formulate diagnosis and ongoing treatment plans including safe discharge based upon this information.
Desirable
* Ability to interpret plain radiographs, scans (commensurate with area of practice)
Experience
Essential
* Evidence of previously working within a leadership role
* Experience in managing acute and chronic cardiac problems, and specialist issues specific to cardiology
* Clinical expertise in end of life care
* Involvement in service improvement
