Main duties of the job
We are seeking a caring, compassionate, and experienced ANP who works within their professional boundaries and scope of practice within the primary care setting. Our ideal candidate will apply advanced assessment and critical thinking skills to decision-making when treating patients following initial history taking and clinical assessment to determine a diagnosis, treatment, and treatment plan.
The Advanced Nurse Practitioner will prioritise and triage the needs of patients making any necessary referrals for investigations in line with local policies, working collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

The purpose of the role is to manage a clinical caseload and deal with a wide and diverse range of health needs in an inner city setting, ensuring the highest standards of care for all registered and temporary patients in accordance with the duties and responsibilities as set out by the Royal College of Nursing.
Job responsibilities
1. Work as part of a multi-disciplinary team to deliver safe and high-quality care.
2. Carry out clinical nursing practice at an advanced level using expert clinical skills, critical judgements & knowledge to deliver appropriate and safe care.
3. Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with acute, long term and complex health needs.
4. Triage and treat patients, performing assessment of their needs and making referrals as necessary to other members of the primary and secondary health care teams as appropriate.
5. Interpret investigation findings to determine effective treatment plans & discuss with GPs where a second opinion is required.
6. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition, using both non-pharmacological and pharmacological treatments.
7. Co-ordinate the planning and delivery of care ensuring patients and carers or relatives are involved where required.
8. Prescribe and review medication that is appropriate for patient needs and in accordance with latest evidence-based practice, national and practice protocols and within the scope of qualified practice.
9. Diagnose and manage acute and chronic conditions, including drug and non-drug-based treatment methods into a management plan.
10. Request diagnostic investigations and interpret within agreed protocols e.g., x-ray, blood tests, scans, etc.
11. Support the management of patients' long-term conditions through offering specialised clinics following appropriate training including (but not limited to) family planning, baby checks, COPD, asthma, diabetes, and anticoagulation.
12. Assess, diagnose and treat patients in practice premises or their own home who require medical attention and/or refer to appropriate agency.
13. Refer patients to other members of the multi-disciplinary practice teams, secondary care organisations and other organisations as necessary.
14. Maintain accurate record keeping on EMIS Web.
