An exciting opportunity has arisen to join the RHS Clinical PICU Pathway Team (6month Secondment)
We are looking for passionate, dedicated and highly motivated individual, to work effectively as part of the RHS Clinical Pathway Team, triaging PICU referrals and be actively involved in the co-ordination and completion of the assessments of referrals to PICU.
The role will include active gatekeeping of all PICU services, identifying appropriateness for admission ensuring the service users are maintained in the least restrictive option for the least amount of time. The role will include the identification of independent sector beds in the absence of Trust provision, liaison with ICB commissioners and other external agencies.
The successful applicant will work alongside the team and through regular monitoring, will support discharge pathways and liaison with all agencies to ensure that potential blockages to discharge are identified and addressed in order to maintain throughput within the service.
Main Duties & Responsibilities• To undertake referral assessment for patients across Pennine Care NHS FT and the wider footprint that may require PICU
• To collaboratively agree and support discharge pathways at point of admission, and work with clinical teams to ensure that the length of a service user’s admission is appropriate to their level of need and associated risk. This will involve working with inpatient services, care coordinator, MDT’s and other agencies.
• To identify cohorts of clients in relation to the identified area of nursing
• To demonstrate an awareness of funding arrangements and the necessary monitoring systems to ensure cost-effectiveness of placements, reviews/external PICU admissions
• To safeguard the confidentiality of information relating to patients
• To seek advice and support from the other members of the PICU MDT’s in relation to providing support to referring teams to prevent admissions to PICU and maintain individuals in the least restrictive placement available in order to safely manage the risk evident at the time of referral.

• To maintain an understanding and awareness of the complex needs of individuals who require PICU
• Fulfil the requirements of the NMC Code of Conduct, Scope of Practice and ensure their application in the workplace
• Maintain up-to-date knowledge and skill in the delivery of PICU services and undertake subsequent development in areas of personal practice
• Identify limits of own knowledge and skill in the delivery of PICU services and undertake subsequent development of these areas of personal practice
• To undertake regular supervision
• To abide by all relevant Trust Policies/procedures and protocols
• To maintain clinically accurate records on Paris, regarding details relating to the management of referrals and the monitoring of out of area placements.
• To maintain effective communication with all other parties involved in the care of individuals whilst placed in out of areas particularly with respect to risk management and safeguarding.
• To undertake face to face assessment and gatekeep all PICU referrals from a variety of sources, inclusive of inpatient settings, custody suites and Prison
• To be a point of contact regarding advice for complex patients and work with teams to develop management plans for patients, in order to maintain them in a least restrictive environment.
• To liaise with family members and involve them in the process of service delivery and planning.
• To work closely with the PICU MDT’s to ensure timely throughput and early identification of any barriers that may affect this.
• To work closely with Team Leaders across the health and social services and other agencies – to develop service and work practice to ensure an effective interface between services which support patient pathways
• To support the assessment of Patient needs in Out of Area PICU and private placements and, together with the Clinical Pathway Team, identify areas within the Trust to support these needs
• To have an understanding of existing PICU contracts with private providers, including oversight and authorisation of any increase in financial cost to the ICB’s g., enhanced observations,
• On request from Commissioners ensure that all clients Out of Area are known to CMHTs and that they all have allocated Care Coordinators and relevant documentation
• To seek advice from other member of the CPT in relation to specialist advice on care pathways and suitable placements to meet the ongoing needs of the clients.
