As a Recovery Coordinator, you will be at the forefront of supporting individuals on their journey to recovery from substance misuse and mental health issues. Working within the Devon area, you will provide personalised support plans, liaise with various stakeholders, and ensure clients have access to the necessary services to aid their recovery. This role demands a compassionate, organised, and proactive individual dedicated to making a positive impact on people's lives.
Responsibilities include, but are not limited to:
1. Conducting comprehensive assessments to understand clients' needs, strengths, and goals.
2. Developing and implementing individualised recovery plans in collaboration with clients.
3. Providing one-on-one support, guidance, and motivation to clients throughout their recovery journey.
4. Maintaining accurate and up-to-date client records and documentation.
5. Coordinating with healthcare professionals, social workers, and other relevant parties to ensure a holistic approach to recovery.
6. Organising and leading support groups, workshops, and educational sessions.
7. Building and maintaining relationships with community organisations, service providers, and local authorities.
8. Preparing and submitting reports on clients' progress and outcomes to management and funding bodies.
Requirements:
1. Proven experience in substance misuse, mental health support, or a related field.
2. Experience in case management and developing personalised support plans.
3. Strong understanding of substance misuse and mental health issues.
4. Ability to manage a caseload effectively and prioritise tasks.
5. Empathetic, non-judgmental, and patient approach.
6. Enhanced DBS, dated within the last 12 months.
7. Willingness to complete an enhanced DBS if you do not currently hold one.
