Job Purpose
Central London Community Healthcare NHS Trust (CLCH) delivers the Stroke and General community in-patient rehabilitation service for Barnet.
The unit provides a comprehensive community-based step-down facility that supports patients’ rehabilitation and recovery in a community setting, with an active therapy programme.
Ongoing assessments for those patients who are medically optimised but who require further assessments outside of the acute setting in relation to their health and social needs and future living arrangements are also undertaken where necessary.
All run a 24/7 service with nurses always present with doctors/therapists available in core hours.
Main duties of the job
* To manage the medical care of patients referred to the ward for rehabilitation. The doctor will be the only medic resident on the ward – there are Consultant reviews twice weekly (Ward round and MDT) and Consultant advice is available by phone.
* Manage any medical deterioration on the ward as appropriate and with the advice of the ward Consultant. (Consultant is based off-site). Patients are required to be deemed Medically Optimised to be transferred for rehabilitation, but due to Frailty and comorbidities may require medical input during their rehabilitation admission. Treatment options depend on severity of issue and include both management on the ward and transfer of care to acute hospital ward/hospice.
* To proactively work to prevent unnecessary admission/readmission to an acute hospital – including daily review of patients on the ward.
* To facilitate appropriate discharge from the Unit to the optimal location for the patient wherever possible including their own homes.
* To support patients, carers and families.

Detailed job description and main responsibilities
* To promote an environment for recovery, ongoing community-based care and assessment with the flexibility to develop the service provision as needs change.
* Clerk in new patients. On admission patients should be reviewed – full medical history should be taken, the patient should be examined, full notes entered to SystmOne and medications prescribed.
* Review of ward patients including those identified by ward doctors.
* Ongoing assessment of patients with minor problems that are manageable in the community.
* The ward doctor will be responsible for making their own contemporaneous notes in SystmOne and written patient records.
* Ordering of investigations such as non-urgent bloods, x-rays, swabs, urine, and other specimen microscopy/culture samples.
* Undertake basic interventions including but not limited to, phlebotomy, insertion of cannulas, insertion of catheters.
Person specification
Education/Qualification
* GMC registered with a licence to practise.
* Completed Foundation Training (FY1 and FY2)
* Two years post-Foundation Training experience
* Up to date Resuscitation and Safeguarding Training.
* Speciality Doctor with training in relevant area e.g., Elderly Care Medicine, Rehabilitation Medicine.
* MRCP, DGM or equivalent
Experience
* At least two years experience working as a Doctor in a rehabilitation setting
* Experience of working in the NHS
* Experience working with patients with frailty
* Previous experience working in a ‘community hospital’, rehabilitation service or frailty service would be an asset
Skills and Knowledge
* Knowledge of GMC ‘Duties of a Doctor’
* Experience of using SystmOne and ICE
