This role provides post holders the opportunity to work across our hospital at home services including our virtual ward, urgent care wrap around service as well as assessing undifferentiated patients. The role will provide post holders with opportunities to develop their experience of urgent and primary care as part of a multidisciplinary team whilst supported by our clinical leads. Main duties of the job
To work as part of the multidisciplinary team, assisting with patient care, administrative and other departmental-related duties, as directed by a registered practitioner to ensure safe, timely and compassionate care for patients.
Under direction of a registered practitioner:
* Assess patients' conditions including observations and other skills including chest auscultation.
* Complete delegated diagnostic tasks such as phlebotomy, urinalysis, bladder scans and ECG recording.
* Where advice is required or patient is outside of scope of practice, or condition warrants escalation link with service duty practitioner.

Job responsibilities
Key duties and responsibilities:
1. Working as part of a team visit patients at home.
2. Assist in delivering a high standard of holistic care to these patients, promoting their equality and dignity at all times.
3. Maintain confidentiality and be polite at all times.
4. Communicate effectively with other members of the team to ensure all appointments are coordinated efficiently.
5. Assist with the preparation, cleanliness and tidiness of all equipment.
6. To ensure professional interactions with patients in person and on the telephone.
7. Accurately enter patient details on all digital systems.
8. Assist with dressings and wound reviews.
9. Supporting and adhering to chaperone policies.
10. Support review of patients by performing and reporting on clinical observation of a patient's temperature, pulse, respiration rate and blood pressure.
11. Undertake chest auscultation as part of patient assessment.
12. Obtain measurement of a patient's height and weight and perform urinalysis if required.
13. Obtain swabs, blood samples, skin scrapings, nail clippings, urine, stool and sputum specimens as requested.
14. Give formal verbal and/or written advice to patients and/or their carers within remit.
15. Liaise closely with the registered colleagues and ancillary staff to ensure the service operates efficiently.
16. Support registered professionals with clinical activities.
17. Report any problems, accidents or incidents to the practitioner in charge and adhere to organisational policy and procedures.
18. Ensure stock levels and equipment are maintained, and exercise care and economy in the use of supplies and equipment.
19. Routine checking of resuscitation and other equipment.
20. Ensure maintenance of good relationships within the department and other areas of the organisation.
21. To attend in-service training as required.
Educational:
1. Undertake and complete relevant competency packages.
2. Highlight additional training requirements relevant to your role, and identify when mandatory training is due for updates.
3. Participate in own appraisal and review process.
4. Keep up to date with current changes in practice sharing the knowledge within the team.
Person Specification
Qualifications
* Ambulance Technician Qualification / Associate Ambulance Practitioner Qualification.
* Driving licence.
Experience
* Proven clinical practice within an acute/urgent care/emergency setting including a minimum of 12 months front line ambulance service experience.
Skills
* Excellent communication skills: interpersonal, written and verbal.
* Confidence to adapt to an evolving and changing service.
* Good organisational skills with the ability to prioritise a demanding workload in order to provide quality care to patients.
