Job summary Trafford Specialist Palliative Care Team The Associate Macmillan Palliative Care Nurse will work as a memberof the wider locality based multidisciplinary Macmillan Palliative CareSupportive Service. The team is very supportive and a fullcompetency-based induction will be offered, supported by a Band 7Clinical Nurse Specialist. The post holder will support Neighborhood teams to deliver locally driven high quality care, responsive to patient and carers' needs. The focus of the multidisciplinary approach will be to support patients and carers to live well, prevent inappropriate hospital admission and meet their preferred priorities for care. Main duties of the job Registered Nurse Up to date registration with the NMC Health Related Degree or equivalent portfolio experience Experience evidenced by completion of specialist short courses, post-graduate training, etc. or relevant Relevant post registration experience Experience in delivering palliative care. Experience of working collaboratively Experience of working within a multi-disciplinary team Experience of working in a community setting. Experience of teaching training others Experience of supporting and supervising other staff in the clinical environment Knowledge of a broad range of assessment tools relevant to cancer/palliative care.
