Care First Recruitment Ltd. is looking for experienced and empathetic Reablement Workers to work with a West London Local Authority with various support units. Locations Hammersmith and Fulham We have other clients/Local Authorities with vacancies Job Purpose: To provide a high standard of reablement care and non-invasive health treatment for clients referred into the Community Independence Service. To work in a multi-agency environment with clients to instil a reablement service focus.To provide care which focuses on the needs of the individual, promotes independence and choice and maximises quality of life. To avoid admissions to more institutional forms of care. To provide a dignified and respectful service that encourages and motivates disabled, frail and older people to live active lives of their choosing. It is important that you have a genuinely caring nature, and the willingness to help the individual overcome their problems, as well as the ability to prioritise your workload. It is common that an individual will be under the care of a multi-disciplinary team, so your ability to pass information effectively between other team members is essential. Working with Clients: To follow reablement goal plans and offer a programme of individualised services to clients including but not limited to: Supporting clients with personal care tasks.This may include activities such as assisting with continence care, washing, bathing, showering, washing of hair, shaving, caring for teeth; transfer from bed to chair and use of special equipment such as bath boards and raised toilet seats; assisting users to dress and undress, eat, engage in social activities. Supporting clients with practical care tasks. Encouraging clients to manage their practical care such as general housework, laundry, making and changing beds and assisting directly when necessary. Encouraging clients to prepare their own nutritionally balanced meals in accordance with dietary requirements and assisting directly when necessary. Supporting clients to maintain and develop social contacts.This may include activities such as advising clients about local leisure voluntary and community services and activities they can engage with. Supporting clients with non-invasive health tasks.This may include activities such as application of support hosiery, application of creams, medication support/ prompts from blister packs, pressure ulcer prevention, changing catheter bags, testing urine and observation monitoring. Supporting clients with therapy interventions such as mobility and walking exercises. To monitor the progress of clients, keeping stakeholders (including key case worker and clinical staff) informed of relevant information relating to progress and changes in the client. To ensure that clients views regarding the service they receive are systematically fed back to the Community Independence Service management team and used to improve the quality of the service. To work in partnership with other teams and agencies within the borough to maximise reablement potential and support the transition into and out of the Community Independence Service To undertake environmental risks assessments and participate in client reviews as required. To attend multidisciplinary meetings regarding the Community Independence Service as required. To ensure that clients are safeguarded at all times within the requirements of the role raising concerns and alerts as appropriate. To observe code of safe practice in respect of possible contact with infectious diseases. To respond to emergencies by summoning appropriate assistance e.g. Ambulance / District Nurse / GP. Maintain the confidentiality of clients at all times To provide direct client health and social care ensuring that their own actions are consistent with clinical governance systems and work within the policies and guidelines To develop skills for working with service users with dementia or other complex needs. n the provision of personalised care, staff may be required to undertake other tasks, not usually considered under the umbrella of care, an example of this may be to assist in the management and care of pets as required. Technology and Communication: To be competent in the use of all relevant technological systems including e-monitoring systems and mobile technology (training will be provided for in-house systems). To formally record client observation activity on appropriate monitoring forms (food and liquid monitoring, temperature, body mass). To provide written and verbal reports regarding the activity undertaken and progress made against goals during supervision, at reviews and when required, utilising electronic and paper-based records To follow the rota communicating any required changes to office staff and relevant stakeholders.Respond promptly to rota changes communicated via mobile technology, verbally or in writing. To communicate appropriately and effectively with other members of the team and to participate in staff meetings as necessary. General: To participate in the monitoring of quality standards such as providing client feedback questionnaire forms and support clients to complete them as relevant. To assist in the induction of new team members with particular reference to systems training, advice and guidance To comply with all Human Resources data recording requirements including sickness returns on resource link and REAL performance reviews. To offer peer support, skills sharing and training within the team as required. To participate in training and make constructive use of supervision and performance review to enhance opportunities for personal development, and to contribute improvements to the service To be available to work flexible shifts. Strong degree of common sense. Whilst there will be training and reablement goal plans to follow, the role is dealing with human beings where common sense and confidence to raise issues and ask for guidance to address matters arising is essential. Ability to understand the process of assessment, planning, implementation and evaluation of care and follow a reablement goal plan. Excellent communication skills. Ability to converse clearly, patiently and sensitively with clients who are from a wide range of cultures and backgrounds and involve them in making decisions about their care. To be able to build and maintain effective working relationships with staff and service users. Ability to accurately interpret and record client information on paper or IT systems Competence with IT skills. Experience gained of caring for vulnerable people, or the ability to demonstrate an understanding of the needs of vulnerable people and the potential to learn how to meet their care needs. A working knowledge of Health and Safety issues and the ability to gain a working knowledge of legislation. Self-motivated, takes responsibility for own actions Hours: 36hrs per week Schedule: Monday to Friday, 08:00 to 16:15pm (1hr lunch break) Ability to Commute to location/service unit: London (required) Work Location: In person Candidates are required to have a minimum of 2 years experience Candidates will compulsorily need to pass a background check by the Disclosure and Barring Service (Enhanced DBS certificate) Pay range: £15 to £20 per hour
