Key Responsibilities:
Innovative Care Coordination: Develop and implement comprehensive care plans tailored to individual needs.
Collaborative Teamwork: Work seamlessly with multidisciplinary teams to deliver integrated and effective mental health services.
Risk Management: Conduct thorough risk assessments and ensure appropriate interventions are in place to safeguard well-being.
Empowering Recovery: Support service users in achieving their recovery goals and maintaining their mental health.
Exemplary Documentation: Maintain accurate and timely records in line with NHS standards and guidelines.
What We're Looking For:
Experience Matters: Proven experience in community mental health settings is essential.
Dependable & Proactive: A reliable and proactive approach to delivering high-quality care.
Professional Qualifications: Registered Mental Health Nurse (RMN), Social Worker, or Occupational Therapist (OT) credentials.
Excellent Communicator: Strong organizational, communication, and interpersonal skills.
Current Registration: Must hold current registration with the NMC, HCPC, or relevant professional body.
