We are seeking to recruit a dynamic, organised and hardworking Band 5 Practice Nurse to join our multi-disciplined, vibrant and fast paced organisation working at Imphal Barracks, York.
As our chosen candidate you will be working as part of a multidisciplinary primary healthcare team, leading the delivery of primary healthcare nursing including elements of occupational health.
This position is being advertised at hours per week and is to be worked over a Monday 08:15-16:30; Tuesday 08:15-16:30 and Wednesday 08:15 - 16:30.

Main duties of the job
Responsibilities include but are not limited to:
8. Providing safe, evidence-based, cost-effective and individualised patient care.
9. Supporting the care of patients with chronic illnesses Asthma, Diabetes, cardiovascular disease and hypertension ensuring continued maintenance of the Long-Term Conditions Register.
10. Assisting with health and well-being clinics (, smoking cessation, weight management, alcohol misuse) using health promotion, health education, screening, and therapeutic communication skills.
11. Organising activities or programmes for patients, drawing on an appropriate range of multi-disciplinary and interprofessional resources within the Medical Facility and across the wider military services to promote integrated and seamless pathways of care.
12. Supporting the development of robust governance systems by contributing to the development and implementation of evidence-based protocols, documentation processes, standards, policies and clinical guidelines through interpreting and synthesising information from a variety of sources and promoting their use in practice.

Depending upon qualifications and experience the post holder will be responsible for the delivery of appropriate assessment treatment, screening and advice to patients within their professional competence and in accordance with DPHC clinical guidelines and protocols.
The successful candidate will need to be able to establish and maintain positive interpersonal relationships with patients and other staff members characterised by trust, mutual respect, and open, honest communication.
