The Transforming Care Autism Team are a specialist assessment and consultation multidisciplinary team who work alongside SPFT colleagues to support discharges and avoid admissions for autistic young people and adults. We have some additional short term funding and we are looking for B6 or B7 Speech and Language Therapist who are interested in working with autistic young people and/or adults who are inpatient or at risk of admission using neuroaffirmative approaches.
Main duties of the job
The Speech and Language therapist role within the team provides specialist assessments of communication and language in order to support a holistic formulation of how neurodiversity may be contributing to presenting risks. Individual interventions may be completed with individuals to collaboratively understand their neurodiversity and make recommendations for communication strategies and reasonable adjustments required.
Job responsibilities
1. Provide a highly specialist Speech and Language Therapy service to autistic people who are on the Transforming Care Risk Register as part of TCAT to support discharge or avoid admission.
2. Provide specialist assessment and contribute to differential diagnosis in the NDS as required by the Principal SLT NDS.
3. Provide highly specialist supervision to other Speech and Language Therapists or Speech and Language Therapy Assistants within Sussex Partnership NHS Foundation Trust, as necessary.
4. Work in collaboration with multidisciplinary colleagues and other agencies.
5. Liaise and consult with other Speech and Language Therapists and/or other professions within mainstream primary and secondary provider services.
6. Work within the MDT to undertake risk assessment, formulation, and management for individual clients with complex presentations and provide advice to other professions on aspects of risk assessment, formulation, and management in relation to communication needs.
7. Prioritise, organise and manage a complex caseload as part of the MDT.
8. Provide highly specialist evidence-based interventions and evaluate effectiveness using agreed outcome measures.
9. Undertake comprehensive assessment of those referred using investigative and analytical skills and formulate individualized interventions based on advanced clinical reasoning and partnership working.
10. Inform and counsel service users, relatives and carers to present complex information clearly and comprehensively to optimise communication.
11. Encourage and enable service users and carers involvement and choice in all intervention options and decisions regarding their care plans/goals wherever possible.
12. Negotiate with clients, relatives and carers and relevant others around care plans and complex case management to motivate and engage them in the therapeutic process.
13. Provide specialist Speech & Language Therapy services for those referred based on national best practice.
14. Be responsible for clinical administration and record keeping related to the specialist caseload according to Trust policy.
15. Be aware of and adhere to Royal College of Speech and Language Therapy clinical guidelines, Health Professions Council standards of proficiency, conduct, performance and ethics, and policies and guidelines relating to specialism.
16. Work within the specialist multidisciplinary team advising on the management of specific Speech and Language Therapy related clinical issues, collaborate in the development and co-ordination of team objectives, and in the development of care pathways and care protocols related to the specialist area.
