Job overview
The South Community Mental Health Service is undergoing an exciting transformation with additional resource allocation to provide ongoing intensive support to the population of Sheffield. This includes working towards a joint approach with Primary Care and the Voluntary Care Sector, reviewing the Community Mental Health Team (CMHT) Service Criteria, and working in discreet small teams in local communities to lead to high-quality care standards.

The successful candidate will be supported within their Care Group through dedicated supervision with highly experienced and motivated team managers, where learning opportunities will be supported and encouraged. From the multi-disciplinary team, we offer regular CPD sessions, and candidates are encouraged to bring their own interests to the team to share and develop their specific interests in mental health.
Main duties of the job
* Provide support to service users working in a discreet Care Group alongside Primary Care Networks in the South of Sheffield.
* Promote positive outcomes by working with people in a proactive, practical sense encouraging service-users' independence.
* Work with your care team to ensure the service promotes healthy lifestyles, opportunities to enhance knowledge about mental health, and provide encouragement to participate in care.
* Liaise with other services to ensure a service user's journey remains as seamless as possible and report back to the MDT in a weekly meeting as required.
Detailed job description and main responsibilities
* Act as a Named Worker for a designated caseload to ensure service users receive a consistent approach and a named person to contact.
* Develop a rapport with service users within appropriate boundaries.
* Encourage service users to be fully involved and engaged with their agreed care plan and to access appropriate services.
* Implement agreed programme of care collaboratively with service users and carers under the supervision of a named professional.
* Help to identify early signs of relapse by monitoring the service user's progress, level of functioning, and mental state.
* Participate in the evaluation of care provided.
* Identify and support carers' needs as required.
* Participate in health promotion including physical health needs.
* Help run social activity groups.

Knowledge and Skills
* A caring and confident attitude.
* Able to work in a multidisciplinary environment.
* Demonstrate an interest in the entire client group.
* Able to respond to the individual rights/needs/opinions of service users.
* Excellent written and verbal communication skills.
* Able to highlight risks in community settings, including those associated with lone working.
* Basic IT skills or willingness to undertake training.
* Able to read, understand and follow care pathways.
* Able to write and record accurate information.
* An interest in and commitment to ongoing development in the role.
* Able to identify personal development needs participating in appraisal and clinical supervision.
* Knowledge of current issues affecting people with serious mental illness and Community Mental Health Teams.
Training and Qualifications
* NVQ Level 3 in Care or equivalent, or willingness to work towards the qualification.
Experience
* Previous experience of working with people across the age range with varying mental health needs.
* Evidence of working effectively as part of a multi-disciplinary team.
* Experience of working in the community/people's own homes.
* Experience of working within acute mental health services.
* Experience of working in Children and Young People’s mental health services.
