About us
Your Health Partnership (YHP) is a cutting-edge Primary Care Network working as part of Sandwell and West Birmingham Hospitals Trust. We pride ourselves on being both innovative and bold as we plan and deliver services to our patients.
YHP PCN was an early adopter of the General Practice Super Partnership Model over the last 10 years, grown to serve around 50,000 patients in our local area. We have worked closely for many years with our local Acute and Community SWB Trusts and in April 2020 vertically integrated to become one of the few Primary Care Directorates in the Country.
Sandwell and West Birmingham NHS Trust (SWBT) is an integrated care organisation with a budget of approximately £600 million and over 7,000 staff. Diversity and social care is at the core of what we do as the Trust provides Community and Acute Services to over half a million people in an urban centre that demands massive regeneration and has substantial premature mortality.
We have three newly emerging strategic objectives:
* Our People - to cultivate and sustain happy, productive and engaged staff
* Our Patients - to be good or outstanding in everything we do
* Our Population - to work seamlessly with our partners to improve lives
