Job summary We have an exciting opportunity for an experienced Practice Nurse to join our esteemed nursing team. This position is offered on a full or part time basis between 15 to 30 hours. The successful candidate will provide high quality patient care, manage their own QOF area and support in long term condition reviews such as Diabetes and Asthma. Main duties of the job Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met. General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition. Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments. Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care. Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad. Running asthma, COPD and diabetes clinics and other chronic disease management reviews. About us We are a large practice located in Warrington serving a population of around 12500 patients. The practice has a friendly, supportive team who work closely this consists of 6 GP Partners, 2 Salaried GP, 3 Advanced Nurse Practitioners, 4 Practice Nurses, 2 Pharmacy Technicians, 2 Healthcare Assistants, 3 Managers and a team of 12 Administrators. We use Systmone/ TPP and experience using this system would be an advantage, but full training will be offered.
