RightCare Barnsley Patient Flow Coordination Teamis expanding, we are looking for new people to join our growing team.
Rightcare provides a single point of contact forhealth and social Care staff across Barnsley for patients requiring hospital orcommunity services. RightCare also takes referrals from Barnsley Hospital wardsand departments for patients requiring discharge support and works closely withthe Hospital Discharge and Patient Flow Team.RightCare also coordinates admissions and discharges for theIntermediate Care Service.
The aim of the service is to ensure that the rightcare is provided, at the right time and in the right place. You will workclosely with health, social care teams and professionals from primary,secondary and community care supporting the RightCare nursing and coordinationstaff access acute care or other more suitable alternatives to hospital admissionthat will meet the needs of the service user.
Main duties of the job
As a Patient Flow coordinator, you will be based in theRightCare call centre and will deal with referrals taken electronically andover the telephone. You will be part of a wider team including Nurses,administrative officers, Social Care and Intermediate Care staff. You will sendreferrals to a range of health and social care providers, including Care Homes,you will follow up the referral to confirm service provision and let therelevant teams know.
We are looking for an individual who is keen to developtheir knowledge and skills in relation to acute and community services, whohave drive and passion and are committed to ensuring that the aims of the RightCareservice is delivered for the benefit of patients and public.
For this role it is essential that you are a confident,organised and have significant experience in the acute and/or community caresetting. You will have excellent interpersonal, communication, decision makingand influencing skill in order to ensure the highest standards of care areachieved. The role is to providecomprehensive and pro-active coordination and support, ensuring that allprocesses and tasks are undertaken efficiently, effectively and to a highstandard, contributing to the smooth running of the service and its overalleffectiveness

Person Specification
Experience
* Previous experience of working in a hospital setting.
* Previous experience of using Medway, Systm1 and other relevant EPR and clinical systems.
Qualifications
* At least 4 GCSEs (Grade A C) or equivalent experience.
* NVQ Level 3 or willingness to work towards.
Knowledge and Awareness
* Able to organise own work and use own initiative.
* Sound understanding of confidentiality.
* Basic understanding of clinical governance.
* Understanding of patient flow.
* Understanding of Community services and pathways.
* Understanding of Discharge to Assess.
