Ranworth Primary Care Network has an exciting opportunity for a qualified clinical pharmacist to join our PCN on a full-time, permanent basis. The role is covering the Ranworth PCN, which is made up of 4 practices and around 31,000 patients in Clacton on Sea and Frinton on Sea. This position is to commence in April 2025.
The post-holder will work across our multidisciplinary team to provide expertise in clinical medicines management, medication reviews, and repeat prescription authorisation. The post-holder will be supported to develop their role to become a non-medical prescriber if that qualification is not already held.
Main duties of the job
* Review the ongoing need for medicine of patients facing long-term conditions, monitoring needs and opportunities to support patients with their medicines ensuring they get the best use of their medicines (i.e., medicines optimisation).
* Make appropriate recommendations to senior pharmacists or GPs for medicine improvement.
* Work with care home staff to improve the safety of medicines ordering and administration.
* Attend and refer patients to multidisciplinary case conferences.
* Management of common/minor/self-limiting ailments.
* Provide a telephone help line for patients with questions, queries, and concerns about their medicines.
* Produce and implement a repeat prescribing policy within each PCN practice.

Person Specification
Qualifications
* GPhC registered Pharmacist.
* Minor ailments certificate.
* MUR and repeat dispensing certification.
Experience
* Experience in managing pharmacy services in primary care.
* Broad knowledge of general practice.
