Making a Difference as an Assessment and Review Worker in Contact Cares, Integrated Discharge Our very busy Integrated Discharge Team based at Whiston Hospital and St Helens Hospital is recruiting for Assessment and Review Worker. The team has a skills mix of staff who work alongside the multi disciplinary team on each ward. The purpose of the post is to assess patients needs, arrange appropriate packages of care, facilitate safe and timely discharges, refer onto voluntary organisations, identify carers needs and signpost to the most appropriate service, for information and support. We will support you with professional supervision, appraisal and training.
