As a Practice Nurse, you willbe a vital member of our clinical team, delivering essential nursing care andsupporting the health and wellbeing of our patients. You will work alongsideGPs, other nurses, and healthcare professionals to ensure the best outcomes forour community.
This part-time post will be up to 22.5 hours per week over 3 days but we have some flexibility with this for the right candidate.
Main duties of the job
Provide general nursing care, including wound management, vaccinations, and phlebotomy.
Perform health promotion and screening, such as cervical cytology, smoking cessation, and lifestyle advice.
Support the management of long-term conditions, such as diabetes, hypertension, and asthma.
Deliver patient education and promote self-management where appropriate.
Assist in the delivery of minor illness and treatment clinics (if trained).
Contribute to the development of care plans and practice protocols.

Job responsibilities
The post holder will use their knowledge of best and evidence based practice and the GMS contract (including the Quality Outcomes Framework) to strive to deliver, promote and improve standards of care.
They will be involved in identifying practice population needs and in being proactive in making suggestions for service improvement.
The post holder is expected to undertake any training and development necessary to keep their knowledge and practice up to date and current.
The post holder will work directly within the Practice Nursing team
Person Specification
Qualifications
* Registered Nurse with a current NMC registration
* Post basic qualifications in primary care related nursing practice including Chronic disease management.
* The post holder must have experience in cervical cytology and immunisations.
* Good working knowledge of infection control procedures, protocols and guidance
* Knowledge of evidence base and current best practice in primary care including health promotion strategies, self-care and prevention
* Knowledge and experience in managing long term conditions
Experience
* Proven experience in a primary care or community setting.
* Excellent communication and interpersonal skills.
* Willingness to undertake further training for the advancement of the service where required
* Experience of working in a GP Practice
* Able to plan and organise, setting and maintaining goals and high standards for themselves and others
* Use EMIS
