An exciting opportunity has come up for a Band 5 nurse with the Tissue Viability Team.
Collaboratively working with our integrated rapid response teams, community matrons, and social service colleagues is essential to ensure seamless care to our patient group.
To provide high quality, skilled nursing care and to assist the Tissue Viability Team Manager in the ongoing planning, implementation, and evaluation of the holistic needs of the patient/client, family, and/or carer. This is a clinic-based role.
Deliver high quality nursing care to housebound patients with complex needs to enable them to remain in their own homes. Working closely with specialist nurses, allied health, and social care professionals to meet the needs of the patients without unnecessary attendance in A&E or being admitted to hospital.
This will involve working independently but as part of the Tissue Viability Nursing Team covering a particular geographical area. This will include first assessments and reviews, planning, implementation, monitoring, and evaluation of individual programmes of care. Candidates will be expected to be flexible to support the corporate approach to patient care as and when required.
