We are ideally recruiting either an Advanced Clinical Practitioner or an Advanced Nurse Practitioner to be part of our extensive multi-disciplinary team based in a GP surgery.
We are unable to offer sponsorship for this role.
To Be a Successful Advanced Practitioner, You Will Need:
* Professional Registration: NMC, HCPC, or GPhC
* Independent Prescriber: Registered as a non-medical prescriber
* Education: Minimum of two Level 7 modules, including clinical assessment skills or MSc (ACP)
* Experience: Autonomous practice in multi-professional teams
* Strong clinical decision-making and assessment skills
Or/ and
* Experienced paramedic (minimum of 3 years within a paramedic role)
* Ideally with some general practice experience or knowledge
We may consider applications from non-prescribers and those who have not yet completed Level 7 module learning or undertaken a full MSc. We can support new to general practice clinicians with training and learning support, however this will be reflected within the salary range.

Main Duties of the Job:
* To be responsible for the delivery of a high standard of patient care as an Advanced Clinical Practitioner or an Advanced Nurse Practitioner in General Practice, using autonomous clinical skills and a broad in-depth theoretical knowledge base.
* To manage a clinical caseload, dealing with presenting patients needs in a primary care setting.
* The Advanced Clinical/ Nurse Practitioner will need to prioritise and triage the needs of patients, accordingly, making any necessary referrals to secondary services for diagnosis and treatment.
* A key part of the role will include carrying out home visits to help manage elderly and frail patients, including those who are acutely ill and those who require proactive care management. The post holder will be expected to support patients to remain well in their own homes to help reduce avoidable admissions. The successful applicant will be expected to work to both a clinic and visit based rota template. As such, a clean, valid driving licence is essential.
* Working hours will be Monday, Wednesday, Thursday 8.30am - 6.30pm and Fridays 9.00am - 6.30pm.

Job Description and Responsibilities:
Primary Duties and Areas of Responsibility:
Clinical:
* Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways, or make necessary referrals to other members of the primary care team.
* Visit patients at home if required.
* Make professionally autonomous decisions for which he/she is accountable.
* Provide a first point of contact within the Practice for patients presenting, both in the Practice, digitally and over the phone, for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem solving and clinical decision making, to establish a diagnosis and care management plan.
* Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings and reports.
* Prescribe safe, effective and appropriate medication as defined by current legislative framework.
* Provide safe, evidence based, cost-effective, individualised patient care.
* Refer patients directly to other services and agencies as appropriate.
* Provision of a holistic service to patients and their families, developing, where appropriate, an on-going plan or care and support, with an emphasis on prevention and self-care.
* Contribute to the development and delivery of specialist primary care services, such as long term disease management.

* Maintenance of professional registration.
* To work within the NMC Code of Professional Conduct and Scope of Professional Practice, and to provide a high standard of professional conduct and nursing care at all times in accordance with the NMC. OR Ensure that professional standards are maintained and within the guidance by the Department of Health, the Health and Care Professions Council (HCPC) and the College of Paramedics (COP).
* Record accurate consultation data in patients records in accordance with the latest NHS guidance and other pertinent standards, ensure the clinical computer system is kept up to date with accurate details recorded and amended.
* Keep up to date with relevant health related policy and work with the practice team to consider the impact and strategies for implementation.
* Work collaboratively with colleagues within and external to the Practice.
* Pro-actively promote the role of the ACP or ANP within the Practice and externally to stakeholders and agencies.
* To encourage and develop teamwork within the Practice.
* Contribute to the identification and management of nursing care risks on a continuing basis.
* Participate in practice meetings.
* Participate in audits as appropriate.
