Job overview
Mid CRHT are currently looking to recruit Band 6 practitioners to join the team. CRHT operates on a 24 hour, 7 day a week basis, providing intensive home support to service users as an alternative to hospital admission. The post holder will be expected to work on a rotational shift basis, including weekends and nights.
Main duties of the job
The post holder is responsible for the assessment of care needs and the development, implementation, and evaluation of programmes of care. Responsibilities include:
1. Managing a case load within the Crisis Resolution/Home Treatment Team.
2. Carrying out generic and specialist assessments, including risk, and providing therapeutic interventions/home treatments.
3. Completing Gatekeeping for all inpatient admission requests.
4. Ensuring effective liaison with inpatient units across the Trust.
5. Participating in a 24hr roster system 7 days a week.
6. Providing face-to-face assessments and acting in an advisory capacity in conjunction with mental health law.
Person specification
Qualifications
* Registered Mental Health Nurse or Dip.SW/CQSW, Dip.COT/SROT
* Evidence of post-registration study/continuing professional development
* PSI Training
* Approved training in phlebotomy
Knowledge/Experience
* Experience of Mental Health assessments
* Demonstrated knowledge of mental health legislation and ECC guidelines
* Experience in crisis intervention
Skills
* Able to communicate effectively
* Possess excellent listening and negotiation skills
* Ability to work as a member of a multidisciplinary team
