Job overview
To provide a defined level of specialist assessment and access to interventions for transgender patients.
The post holder will be part of the transgender health team and act as a Transgender Healthcare Professional, working as part of the service to support patients in line with the NHS England national service specification.
The role requires the post holder to be a named professional for patients and undertake both assessment and follow-up responsibilities for an identified caseload of transgender and non-binary patients.
The role involves monitoring cross-sex hormone treatment for trans male, trans female, and non-binary patients.
The post holder will be part of the team providing a service to patients, and on occasions their families, including direct and indirect clinical work that supports the Clinical Lead and Service Manager in developing the service.
To demonstrate active clinical and managerial leadership skills within the team, ensuring that all team members receive appropriate clinical/practice supervision and that professional standards are maintained.
Through annual appraisal, ensure that mandatory training, practice education, and development are actively identified for all staff, and feed into the Trust’s continuous professional development planning process.
Lead by example, motivating and empowering others and promoting positive attitudes, mutual understanding, and collaboration between all services involved in the delivery of the service.
Main duties of the job
1. To assess, design, implement safe, creative, and effective treatment plans in collaboration with the patients’ needs.
2. Manage a diverse caseload of patients ensuring a high standard of clinical care.
3. To work as an independent Non-Medical Prescriber within this field.
4. Work as a team member being flexible to the needs of the team.
5. To maintain accurate, timely, and relevant clinical records both written and computerized.
6. To maintain the safe custody of healthcare records.
7. Support the implementation of team objectives within the multidisciplinary specialist team.
8. To maintain NMC registration and adhere to the revalidation requirements.
9. Maintain adequate patient documentation to NMC requirements for all patients seen and advice given in any practice setting and contribute to clinical activity/data collection, as required.
10. To provide appropriate and relevant support, where appropriate to peers and non-registered team members.
11. Compliance with best practice directions (WPATH and NICE) and participation in quality initiatives.

Detailed job description and main responsibilities
The Nottingham Centre for Transgender Health is a large transgender health service based in Nottingham but is part of a wider Network that includes the East of England Service, Indigo in Manchester, and the Sussex gender service. The Network provides high-quality assessment, referrals for treatments, case management, and GP support.
Patients will receive a blended model of face-to-face and video consultation appointments with the clinics running 5 days a week. It is a busy working environment which requires high levels of confidentiality and personal skills.
As one of the team, you will undertake the provision and planning of assessment, care, and treatment for transgender patients, whilst having direct face-to-face and video work with patients and undertake independent Non-Medical Prescribing. You will be fully supported by experienced transgender health professionals who will work with you to provide a seamless gender service and pathway.

* Significant further specialist knowledge/post registration experience to Master level or equivalent underpinned by theory within physical healthcare or other formal training and leadership/management
