Position Title: Primary Case Manager Position Status: Full Time Program: Crisis JOB SUMMARY The Primary Case Manager provides needs assessments, communication with referral sources, assisting clients with managing case with insurance company/JBFS. Scheduling and managing all medical appointments, linkage to community resources to meet client needs. Utilizing internal tracking processes and following Standard Operating Procedures to ensure best outcomes. ESSENTIAL FUNCTIONS Essential functions are duties which are essential and primary to the position or the reason the position exists. The Primary Case Manager is an integral role in carrying out the mission of The Counseling Center, Inc., by performing the following duties: Assesses needs of client, assists in development of treatment plan, and gathers information upon admission to ensure communication with referral sources, medical providers and payee's are completed in timely manner. Assists in obtaining supportive services to aid the client in their personal goals in recovery through the services of the agency or through referral to other agencies or programs. Assist clients in accessing community resources and provides supportive services. Provides group and individual services in an effort to improve coping and daily living skills in an environment that is conducive to recovery efforts. Provides written documentation of client services in compliance with agency, OAC and compliance department standards. Case Manager will assist in the development of treatment plans, weekly communication with referral sources, and any other vested party. Initiates all correspondence and paperwork related to needs assessments and client service reports (i.e., letters, progress notes, rosters, trackers, mailing lists) Attends weekly clinical staffing to assist with the discharge planning for aftercare, communication with referral sources, JBFS/insurance companies, and make referral. SECONDARY FUNCTIONS Secondary functions are duties, which are not exclusive of the position, can be performed by other positions; however, secondary duties are to be performed for the efficiency of The Counseling Center, Inc. Performs other duties as assigned by the Program Coordinator/Director. COMPETENCIES Demonstrates Knowledge in Substance use disorders, community psychiatric supportive treatment, ASAM LOC, medical necessity. Demonstrates understanding of clinical practice and performance including understanding of the process of assessment, treatment, and confidentiality. Ability in the area of effective communication, interpersonal skills, written communication Ability to complete all documentation in accordance with agency policy. Effective time management/organizational skills. MINIMUM QUALIFICATIONS, INCLUDING TRAINING AND EXPERIENCE High School Diploma or equivalent, Behavioral Science Degree preferred. CDCA Required Knowledge in Substance use disorders, community psychiatric supportive treatment, ASAM Level of Care and medical necessity. 12-Step Recovery knowledge Valid Ohio Drivers License and ability to maintain insurability preferred. Maintain CPR/First Aid treatment preferred. Maintains applicable agency trainings, attends weekly supervision.
