Job summary To work as an autonomous practitioner, responsible for the delivery of total patient care, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The Role will encompass working across four of the embedded sites within the Heart of Lincoln remit. The Nurse Practitioner / Advanced Nurse Practitioner / Advanced Clinical Practitioner is expected to, within their professional boundaries, sees patients on an appointment basis both on the day and pre planned care appointments, provides care including initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients presenting at the practice. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.
