Your daily tasks will be varied and include supporting people with activities of daily living, personal care, moving and handling, prompting medication, meal preparation and social care and support, working with therapists and social workers to make sure people are achieving their recovery goals. Care is provided to patients within their own homes. About the Community Integrated Response Team (CIRT) CIRT is an integrated team of clinical staff and professionals including; Nurses Physiotherapists, Occupational Therapists and Social Workers. The team have a strong focus on admission prevention and early supported discharged from ED and assessment areas.
