CLINICAL PHARMACIST INDEPENDENT PRESCRIBER (GP PRACTICE) 8H*3D / Week
The position requires a qualified pharmacist with an independent prescribing qualification, who will operate within their professional limits as a member of a multidisciplinary team that includes GPs, ANPs, Physician Associates, Paramedics, and First Contact Physiotherapists.
Main duties of the job
The successful candidate will collaborate closely with the GP and other practice clinical and non-clinical staff, playing a key role in supporting GPs and the broader primary care team to provide safe and effective care. They will provide medication-related patient services, including direct patient care. As a leader, the candidate will guide and support the practice team to ensure safe and effective prescribing, while maintaining and enhancing the quality of patient care within the practice.
Job responsibilities
Consult with patients who have one or more medical conditions that require medication optimization (e.g., COPD, asthma, hypertension).
Review the ongoing necessity of each medication, assess monitoring needs, and provide support to patients in managing their medications, ensuring they achieve the best outcomes from their treatments (i.e., medication optimization).
* Provide GPs with recommendations for medication improvements.
* Conduct clinical medication reviews with patients and offer recommendations to the senior clinical pharmacist, nurses, and/or GP regarding prescribing and monitoring.
* Respond to medicine-related inquiries from GPs, practice staff, other healthcare teams (e.g., community pharmacy), and patients.
* Offer solutions and recommendations.
* Follow up with patients to monitor the effects of any changes.
* Review the use of medications commonly linked to unplanned hospital admissions and readmissions through audits and individual patient reviews.
Implement changes to medications in response to MHRA alerts, product withdrawals, and other local and national guidelines.
Implementation of Local and National Guidelines and Formulary Recommendations
* Monitor practice prescribing in accordance with the local formulary and recommend to GPs which medications should be prescribed by hospital doctors (red drugs) or be subject to shared care (amber drugs).
* Audit the practice's compliance with NICE technology assessment guidance.

Qualifications
* Degree in Pharmacy with Independent Prescribing Qualification
* Ability to convey complex and sensitive information clearly to various audiences, such as patients
* Proficient IT skills
* Capable of obtaining and analyzing complex technical information
* Ability to work under pressure and meet deadlines
* Produce timely and informative reports
* Work effectively both independently and as a team member
* Self-motivated
* Adaptable
* Past experience of working in a GP practice
